Hold L2 + R2 and press Square(3), Triangle(2), X(2) then release L2 + R2. A cheat menu will appear, and you should have all the costumes for create a player mode.

If you get called for a goaltending foul, quickly press L1 + R1 then Triangle and you should get the ball back.
